Therapists provide talk therapy and counseling for mental health, emotional, behavioral, and relationship concerns. They work with individuals, couples, families, and groups, using a range of evidence-based approaches such as cognitive behavioral therapy, family systems therapy, and other modalities. Therapists do not prescribe medication; patients who may need medication are typically referred to a psychiatrist or psychiatric nurse practitioner for that part of their care.
"Therapist" is an umbrella term covering several distinct licensed credentials, each with its own training path. Licensed Psychologists hold a doctoral degree (PhD or PsyD). Licensed Clinical Social Workers (LCSWs) complete a master's in social work accredited by the Council on Social Work Education. Licensed Mental Health Counselors (LMHCs) complete a master's in counseling, typically accredited by CACREP. Licensed Marriage and Family Therapists (LMFTs) complete a master's specialized in relational and family therapy. Psychiatric/Mental Health Clinical Nurse Specialists hold an advanced nursing degree. All require state licensure, several thousand hours of supervised post-graduate clinical experience, a national examination, and ongoing continuing education.
A first visit with a therapist typically begins with an intake conversation covering presenting concerns, mental health history, relevant medical history, and current support systems. The therapist may use structured screening tools alongside the conversation to assess symptoms and treatment needs. Together, the patient and therapist usually discuss treatment goals, the therapist's approach, expected frequency of sessions, and practical matters such as confidentiality, fees, and insurance.
The mental health professions overlap significantly in the talk therapy they provide, but differ in training emphasis. Psychologists are trained in psychological assessment and research-based therapy. Clinical social workers are trained to integrate psychotherapy with attention to social and community factors. Mental health counselors focus on individual and group counseling techniques. Marriage and family therapists specialize in relational dynamics. Psychiatric nurse specialists bring a clinical nursing background, and in some states have prescribing authority.
